Domestic Violence Outreach Program

The Family Tree offers individual advocacy, support, and safety planning services for people experiencing domestic violence. They cannot provide clinical counseling services, but can refer individuals out who need them.

Survivors of Suicide Peer Support & Outreach Program

The Survivor Outreach Program is a peer-to-peer support service for people who have lost a loved one to suicide. People who are interested in recieving support fill out a questionnaire about themselves and the loss that they experienced so they can be matched with a volunteer peer who has had a similar experience. Peer support can be offered in person on a limited basis, but most support is offered via telephone, video call, or email.

Jefferson Center for Mental Health Navigation Services

The Navigation Services Team at Jefferson Center for Mental Health assists community members with finding public benefits for which they are eligible and assists them in the application process. They also provide information & referrals to community resources and programs.

Infant/Toddler Loss Peer Support

One-on-one emotional encouragement and help navigating recovery from people with lived experience of infant/toddler loss. Peer Contact Request Form and Authorization for Release of Information (available on the webpage) required.

Jefferson Plaza Family Integrated Care Clinic

Jefferson Plaza Health Home offers pediatric & adolescent physical healthcare, immunizations, prenatal care, well visits, physicals, chronic disease management services, mental health services, substance abuse treatment, and resources/referrals in one location.
